Request to be on agenda

Click on the agenda form link (this is a Microsoft® Excel Spread Sheet), fill in the required information, and then save the file to your own computer in the directory of your choice.  Once the information has been saved you can either mail, fax or email the file as an attachment.

Please use this form when asking to be placed on any future agenda.

Requests to be placed  on the board's agenda should be made by filing your request so it is received in our office by fax or email prior to 8:00 a.m. the Friday preceding the boards meeting.
